0165(2)(c) in the science/art room, an electrical cable is accessible to childcare children inside a box. In the large pre-school room, an phone charger cable is not secure and long enough that can form a loop around a child's neck. 0165(3)(a) in the outdoor play, the soil table has two small screws and in the pathway to the outdoor a mirror is accessible to childcare children. 0165(3)(f) in the large pre-school room, art supplies shelf is not secure to the wall and three medium flowerpots on top of the shelves; in the science/art room, a medium flowerpot is on top of the shelf; and outdoor area, the large canopy is not anchored to the ground; all these items have potential to fall on childcare children. 0165(3)(g) in the science/art room has two shelves with chipping paint; in the large pre-school room has three shelves in the block area with chipping paint; in the literacy room, blue paint wall has some chipping paint, broken chair and shelf has tape residue, in the jumping room has wall paper is peeling; in the pathway going to the outdoor space, the carpet is peeling and a broken bike; and in the outdoor, the concrete wall has a small hole, playhouse has a broken board, the purple arch has flaking paint, two carpets were wore out around the edges and four tires were accessible to childcare children.
